Insurance coverage is accepted by these carriers: Medicare, Medicare Advantage/Plus Blue, Blue Cross Blue Shield/PPO/FEP, Blue Care Network/Advantage, Blue Cross Complete, Humana, Aetna, United Health Care, Theramatrix, Priority Health, HAP Cigna, PPOM, Auto Accident Insurance

Please have your new patient forms completed, prior to arrival. IF you do not have your completed forms, Please arrive 15 minutes before your first visit, to allow time for new patient forms. Your first visit will last 1 hour. Each visit after that will be 45 minutes to one hour.
When you come for your first visit, please bring the prescription/referral for Physical Therapy that your physician issued. Also be sure to bring your insurance card, picture ID/driver’s license and any reports(MRI, or X-ray) results you may have.

Usually your insurance will cover your physical therapy completely. However, this does not mean you will not have fees to pay. In some cases there are co-payment and co- insurance amounts that will be charged to you.

We suggest you dress in comfortable, loose fitting or exercise clothing, tennis shoes, or appropriate footwear.
During your first visit a Physical Therapist will evaluate your condition or injury, establish goals, individual needs, and lifestyle. Then we will formulate your customized treatment plan, tailored to help you heal as effectively as possible.
